Cincinnati, Ohio, Ohio, The Cincinnati, OH VA Medical Center is seeking a highly experienced RN to serve as our Outcomes and Performance Manager. The O&P Manager will be expected to operate directly under the Chief of Quality, Patient Safety, and High Reliability(QPSHR), taking action to develop or modify objectives and boundaries of assignments as indicated, and play a critical role in organizational performance improvement as a member of the interdisciplinary team. Graduation from School of Nursing: Graduate of a school of professional nursing approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant. On and after [March 17, 2009], registered nurses appointed or otherwise moving into these assignments must meet and maintain the following additional qualifications. This includes employees appointed before [March 17, 2009], who obtain such qualifications on or after [March 17, 2009. ] Nurse Practitioners. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master’s degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. (The certification must be in the specialty to which the individual is being appointed or selected. ) Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I, Level I – An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I, Level II – An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor’s degree in a related field with no additional nursing practice/experience; OR a Bachelor of Science in Nursing (BSN) with no additional nursing practice/experience. Physical Demands: Per VA Directive and Handbook 5019 – a physical examination will be conducted based on the physical requirements of the position. A physical examination prior to placement is required. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.

The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles. BLS/ACLS Requirements: Current certification in BLS is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high-risk areas (SICU/ICU/CCU/MICU/PACU/ED/and all procedure suites using moderate sedation, etc. ). The Clinical Outcomes and Performance Coordinator is responsible for planning, designing, integrating, implementing, modifying, and administering the Quality, Patient Safety, and High Reliability Strategic Plan to monitor, analyze, and continuously improve the quality and safety of veteran care across services. The Outcomes & Performance Manager operates under delegated authority of the Chief of Quality, Patient Safety, and High Reliability (QPSHR), taking action to develop or modify objectives and boundaries of assignment as indicated, and plays a critical role in organizational performance improvement as a member of the interdisciplinary team. The incumbent collaborates with medical center leadership, Risk Management, and staff to proactively identify areas of opportunity and to develop and implement remedies to improve service and organizational performance. The incumbent coordinates and ensures timely project teams to improve organizational performance and Veteran outcomes. Core elements of performance include clinical competency combined with a high degree of leadership, motivation, self-direction, teaching, and initiative in quality of performance management activities are also required. Major Duties include but are not limited to: Demonstrates expertise in performance improvement and change management concepts of the organization, VHA, and NCPS national mandates Maintains working knowledge of evidence based best practices Consults and provides clinical and performance recommendations regarding quality, safety, and efficiency with the goal of continuous improvement in patient care delivery across the healthcare continuum Maintains and demonstrates a systems perspective when consulting and recommending improvement actions Attends educational offerings to ensure current knowledge of performance improvement practices Identifies facility needs related to quality and systems redesign, initiates and advises subject matter expert groups in development of action plans and programs for improvement of outcomes and performance measures Develops complimentary training when user specific needs are identified, to enhance comprehensive understanding of quality standards and practices Establishing and maintaining processes to assure the effectiveness of performance improvement systems to facilitate comparability in the quality, appropriateness and timeliness of patient care and services across the continuum. Identifies innovative or promising local quality management and patient safety improvement initiatives and facilitating deployment, as appropriate, to enhance care and services and minimize clinical risks to patients.
