Madera, California, Case Manager – Registered Nurse – Part Time/Days SUMMARY: This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services available to meet a patient’s health needs. Coordinates services provided to a specified group of patients, ensuring that the care is provided in an appropriate setting, and that the severity of illness and the intensity of service is consistent with the patient’s clinical condition. Collaborates with the health care team members to ensure that the care provided meets desired patient outcomes with appropriate utilization of resources within expected time-frames across the continuum of care. This position practices within the scope of the Nursing Practice Act, and in accordance with the policies on Valley Children’s Healthcare. REQUIREMENTS: This position requires an Associate’s Degree in Nursing with a Bachelor’s Degree preferred. A valid California RN license is required as well as two (2) years nursing experience. Pediatric experience and Case Management/Utilization Management experience is desirable. Bilingual skills are preferred. POSITION DETAILS: Part Time, 40hours per 2-week pay period. Day shift, weekend call required on rotation PAY RANGE: $48.
