We are seeking a Case Management Manager to join us at South Shore Hospital in Weymouth, MA. In this position, you will provide leadership to the Case Management staff in the daily operations of the Case Management Department, train and mentor all Case Managers and provides patient-focused case management, discharge planning and coordination of care that supports quality, utilization and cost of care and operations.
 
Responsibilities:

Develop performance goals and objectives for the Case Management department
Participate in the interviewing process and recommend selection of staff, direct activities and evaluate professional and support staff
Develop, analyze and adhere to departmental capital and operating budgets, ensuring appropriate staffing and resources are in place to support the case management standards of practice
Provide for the effective utilization of resources (staff, financial material and equipment) to meet established goals and objectives and act as a resource to Case Managers and support staff
Act as a clinical consultant/practitioner for all members of the healthcare team regarding complex issues in areas of case management, such as hospital location, appropriateness of admission and continued stay denials
Develop and implement a site-specific quality improvement plan to continuously evaluate the effectiveness of the case management program.

 
Qualifications:

Current RN license to practice in the state of Massachusetts
Community health nursing and discharge planning experience preferred
Demonstrated leadership skills and ability to work in a team environment
Proficient computer skills and knowledge of data collection and analysis for reporting purposes strongly preferred
Excellent written, oral and interpersonal communication skills
