About the Role: Job Summary Maintain the integrity of the master patient index and facilitate synchronization of all impacted databases throughout the organization. When duplicate patient information is identified, assure the medical record content is promptly and accurately merged to facilitate patient care. Responsible for oversight and integrity of patient data, as well as day to day clean up and correction activities in the system-wide EHR. Assists with chart corrections, including contact moves, intermixed records, and/or any identified Chart Correction issues, using the Chart Correction workspace. Minimum Job Qualifications Licensure or other certifications: LPN. Educational Requirements: Associates Degree. Minimum Experience: Minimum three (3) to five (5) years experience working in a clinical setting required. Minimum six (6) years experience working with Healthcare applications and Health Information Management required. Other: Preferred Job Qualifications Preferred Licensure or other certifications: Preferred Educational Requirements: Four (4) year degree from an accredited college.
Six (6) years minimum experience in Revenue Cycle applications or Healthcare IT is acceptable in absence of four year degree. Preferred Experience: Educational background and/or leadership role a plus. Certification in Epic Identity a plus. Must be able to determine root causes for data integrity issues Essential Tasks and Responsibilities Maintains the integrity of the electronic master patient index and facilitates synchronization of all impacted databases throughout the organization to ensure patient safety. Serves as subject expert for EHR oversight and integrity of patient data, as well as day to day clean up and correction activities in the system-wide EHR.
Identifies duplicate medical record numbers within the system-wide EHR as appropriate and according to established policies & procedures, using the Duplicate Patient Report and Mark for Merge Report. Assists with clinical corrections and reviews requiring a licensed caregiver for approval (i. e. Orders, medications, etc. ) to make CSN/Account corrections, overlays, and/or any identified Chart Correction issue, including contact moves, using the Chart Correction Workspace. Merges all duplicate medical record numbers and other associated patient information maintaining the accuracy and integrity of electronic and manual records. Validates complete clinical information is only available within the surviving medical record number. Assists with CSN/Account corrections, overlays, and/or any identified Chart Correction issue, including contact moves, using the Chart Correction Workspace. Designs and assists with education of the teams that impact the MPI and the HIE coordinators to facilitate and assist in reduction/elimination of MPI duplicates through identification of root causes of data integrity issues and suggesting processes and procedures that will increase data integrity. Provides timely responses to Data Integrity/Chart Correction phone calls and general e-mail/inbox questions.
Facilitates identification and resolution of incidents where an individual is identified to have more than one medical record number to prevent potential patient safety issues. Resolves duplicate medical record numbers identified outside of the registration process and notifies other departments of duplicates and surviving numbers to assure synchronization of indices throughout the organization. Promptly notifies clinicians and departments treating an inpatient with duplicate medical record number(s) of the duplicate medical record number using “Mark for Merge” patient highlight to assure access to all clinical information for patient care. Sends advanced email notifications to clinicians in ambulatory, LTC, and Hospice prior to merging duplicate medical records containing recent encounters in these treatment settings. Monitors failed merges to assure quick and accurate resolution of failure, and resubmit merge. Provides and assists management team with various reports, statistics, and communications. Assists with education and daily functions to ensure the accuracy & integrity of our EMPI. Responds to Data Integrity phone calls and general e-mail/inbox questions.
