Nampa, Idaho, Overview St. Luke’s Health System in Nampa is seeking a Mental Health Therapist to join our new Community Clinic team. About the Community Clinic: The South Nampa Clinic will use consumer insights to shape the care environment; therefore, a flexible mindset to adapt to consumer needs and preferences is required. The clinic is open 7 days a week (M-F 8:00 a. m. -8:00 p. m. ; Sa-Su 9:00 a. m. -5:00 p.
m. ), and is comprised of one clinical team that:
* Serves pediatric, adult, and senior populations.
* Provides front door care to include wellness, acute, behavioral health, diagnostics, and pharmacy.
* Ensures workflows and cross-training is built to minimize transitions during care.
* Observes, captures, and applies insight and findings from our consumer, the community clinic, comparable clinics, and other markets and consumers to recommend explorative studies and/or new standards. About the role:
* You will work in a different model of primary care that evolves with the market and consumer needs.
* You will assist with both scheduled and unscheduled patient needs/appointments.
* You will assist with the development, refinement, and enhancement of clinical programs, initiatives, processes, policies, workflows, and projects. Expect collaboration, shared decision making, and partnership with care team members.
* The therapist will be responsible for patient care services including, but not limited to: o Providing crisis intervention, risk assessments, and safety plan interventions to patients.
o Completing behavioral health diagnostic evaluations and determining treatment needs. o Provision of evidence-based, short-term therapy services to patients in an individual, family, and/or group setting. o Provision of treatment to assist patients with medical conditions that would benefit from behavioral changes. o Collaboration with other staff and providers – in the clinic, throughout St. Luke’s Health System, and in the community. o Providing community resource information and referrals; care management as needed. o Examples of issues that the therapist may assist with: depression, anxiety, insomnia, stress management, substance use, grief, suicidal ideation, recent psychiatric hospitalization, chronic pain/illness, difficulty coping with medical illness or new diagnosis, treatment adherence, uncontrolled medical issues, obesity, unexplained symptoms, etc.
* You will utilize our EMR (EPIC) to collect data, document patient interactions in the field, organize information, track tasks, and communicate with your team, patients, and community resources. Responsible for the planning and provision of mental health treatment services to patients in an individual, family, or group setting.
Responsible for the onoing management and resassement of the treatment plan, which may include a readjustment of prescribed treatment or goals, and transisition or termination planning. Provides crisis intervention services to patients, families, and staff. Coordinates with and supports medication management while monitoring treatment adherence, including communicating with the prescribing provider any side effects and potential complications. Provides community resource information and referrals. Eligible to participate in Third Party billing. Completes documentation in accordance with current policy and requirement for care delivery setting. Other duties and responsibilities as assigned. Qualifications Education: Master’s degree in social work, counseling, marriage and family therapy or related field. Experience: None Licenses/Certifications: American Heart Association (AHA) BLS Provider certification and hold a License in the State of Practice in one of the following: Licensed Professional Counselor (LPC), Licensed Masters Social Worker (LMSW) or Licensed Associate Marriage and Family Therapist (LAMFT) Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) Why St. Luke’s A strong, talented staff is at the heart of St.
