Ventura, California, Position Overview:
The Director of Patient Safety and Clinical Risk Management is responsible for planning and implementing the Patient Safety Program across the system. Activities include, but are not limited to: managing and analyzing event data, conducting educational programs, directing service recovery, leading investigations and root cause analysis, conducting the Culture of Safety Survey and creating an action plan to address results, providing direction and support for disclosures and CANDOR, complying with risk management-related standards of accrediting and regulatory agencies with the objective of enhancing patient safety, promoting high quality care, and minimizing loss. This individual develops policy and/or organizational changes.
Essential Functions and Responsibilities:
Review and analysis of notification reports and occurrences, including adverse/sentinel events. Conducts event investigations and root cause analyses in response to significant safety events. Collaborates with managers and staff in the development of risk reduction action plans. Conducts patient safety educational programs as needed. Coordinates the Communication and Optimal Resolution program to assure proper and timely disclosure while supporting staff. Manages the incident reporting and document control system.
Leads Patient Safety Program as defined within the Quality Management-Performance Improvement and Patient Safety Plan: Participates in the Safety Committee, implements patient safety initiatives and practices to continually improve patient safety and prevent harm. Annually assesses outcomes for inclusion in Quality Management-Performance Improvement and Patient Safety Plan appraisal and identifies areas for focused improvement.
Manages the Patient Experience program across the system to continually enhance patient satisfaction and their experience with our system. Works with the survey vendor to maximize survey responses and data collection and utilization. Assists with the Patient Experience Committee and Patient Family Advisory Council.
Manages patient grievances: Works in coordination with hospital staff to resolve complaints before they develop into professional liability claims; ensures required response to patient is complete and timely; analyzes trends; develops and presents reports to administration, directors, medical staff and the Board;
Regulatory Compliance: Assesses and implements processes to ensure compliance with regulatory and accreditation requirements; reviews and updates policies and procedures to ensure consistent with current practice and requirements; educates staff and medical staff members as needed. Evaluates the Patient Safety Program on an annual basis. Revises Plan as needed.

Supervisory Responsibilities: Supervises Manager, Clinical Risk Management and Patient Safety and P atient Experience Coordinator. Required:
Bachelor’s Degree in Nursing (BSN) or clinical field
Five (5) years clinical experience in an acute care setting
Three (3) years risk management experience
Current Certified Professional in Healthcare Risk Management certification (CPHRM)
Current Certified Professional in Patient Safety (CPPS)
Preferred:
Current Registered Nurse License
Master’s degree
The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Most often, a newly hired employee will be placed below the midpoint of the range.
