Kingston, New York, Job SummaryUnder the general direction of the Director, Health Information Management, serves as the primary resource for the Electronic Medical Record System. Responsible for preparing/ prepping documents into the OneContent/ electronic document imaging system. This position will maintain daily workflow, insuring that documents are prepped in a timely and efficient manner to avoid delay in further processing. Examines pages and verifies patient information on each page of the document. Responsibilities Confirms patient name, medical record number, and account number on every page in the patient medical record, front and back to ensure that each page in the record is for the same patient and encounter. Affixes missing patient label in the addressograph portion of the page Identifies and tapes any torn edges of the document Removes all staples Mounts and tapes down any sheets less than 8. 5x 11 inches on an 8. 5×11 inch mounting sheet. Tapes down sheets with rhythm strips or other mounted documents so that the top of the strip does not catch on the automatic document feeder. Arranges Physician Orders, progress notes, and nurse’s notes in chronological order Ensures all pages are in the same and proper orientation.
Accurately completes a batch cover sheet for each batch prepared. Items to be completed on the sheet include the patient’s name, batch label #, batch I. D. #, rescan batch ID# (If necessary), batch type, patient type, prep date and initials Scans and indexes records as needed after prep. Processes release of patient information in accordance with hospital policy and procedures, state and federal requirements. Education: High School Graduate or GED equivalent required. Experience: Prior experience working in an acute care hospital medical records department preferred. Ability to perform job function and make decisions without direct supervision. Training/License: None Specified Other: Must be able to speak English and have good oral and written communication skills Excellent keyboard skills Must be detail-oriented, display a high level of accuracy, and be organized. May be required to work evenings and weekends.
