Full – Time: 40 Hours Per Week 1st Shift: 8am – 4:30pm Monday – Friday Hybrid: Remote and onsite requirements Summary: The HMG Service Coordinator recruit and orient prospective Help Me Grow families. Responsibilities: 1. Recruit and orient prospective Help Me Grow families. 2. Make regularly scheduled home visits to work with parents and children on development.
3. Assure that all developmental screenings are completed and refer for a developmental evaluation if a delay is suspected. 4. Facilitate and participate in the development, review, and monitoring of the IFSP to ensure family outcomes. 5. Identify specialized services and other providers and inform families of advocacy services. 6. Coordinate and monitor the delivery of services. Coordinate with medical and health providers. 7.
Coordinate transition to other programs and services. 8. Assure all required information is entered into web-based Early Track system. 9. Assures all required information is entered into the Help Me Grow record and all Help Me Grow forms are completed. 10. Participate in weekly individual and team supervision. 11. Participate as a member of the developmental evaluation team to assist in determining eligibility for Part C services as necessary. 12.
Demonstrates the knowledge and skills necessary to provide care for the physical, psychological, social, educational, and safety needs of the patients served ages 0 to 3. 13. Other duties as required. Other information: Technical Expertise 1. Experience working with parents and children [0 to 3 years of age] is required. 2. Experience working with culturally diverse individuals is preferred. 3. Experience with community resources used by families with children is /preferred. 4.
Travel and valid Ohio driver’s license is required. 5. Experience working with all levels within an organization is preferred. 6. Experience in healthcare is preferred. 7. Proficiency in MS Office [Outlook, Excel, Word] or similar software is required. Education and Experience 1. Education: Bachelor’s degree in Social Work, Early Childhood Education, or related field is required Master’s degree is preferred. 2.
Certification: Service Coordinator Credential is required within one year of hire. 3. Years of relevant experience: 3 to 5 years is preferred. 4. Years of experience supervising: None.
