Schedule: As Needed Shift : Every other weekend 12:30pm-9pm Location: AdventHealth GlenOaks The role you’ll contribute: Obtains and documents through the electronic medical record (EMR) both timely and accurate medication histories for patients using multiple data sources. The value you’ll bring to the team: · Interviews patients, caregivers, or providers to obtain the most accurate medication history based on patient-specific factors (eg , illness, cognition, literacy). · Accurately uses and communicates with external resources to obtain pertinent information. Documents medication history in the EMR. · Provides appropriate escalation and appropriate handoffs with complex or incomplete medication histories.
Effectively communicates with other members of the health care team. · Maintains knowledge regarding new medications and dosage forms that may be used in an outpatient/ambulatory setting. · Monitors multiple patient queues from facilities or EMRs. Qualifications The expertise and experiences you’ll need to succeed: · High school diploma/GED with 2 years of experience, or Associate’s degree, or Technical degree required · Pharmacy Tech credentialed from the Pharmacy Technician Certification Board (PTCB) preferred. · Pharmacy Tech credentialed from the National Healthcareer Association preferred.
