About This Position ChristianaCare is looking for a Peer Case Manager to be embedded within the New Castle County Division of Police Behavioral Health Unit; Hero Help Addiction (HHA), which is a liaison among providers, hospitals, peer counselors, and people seeking treatment for substance use disorder (SUD). The Peer Case Manager will deliver case management, outreach, individual, group, and psycho-education services to adults with substance use disorder including opioid use disorders to ensure that they receive appropriate and immediate access to substance use disorder treatment by connecting them to a treatment provider and addressing any barriers they may have to pursuing treatment. This position will deliver a wide range of clinical and support services and will be a part of a team that works closely together. Some responsibilities include: Delivers outreach, case management, referrals, individual motivational interviewing, and assessment services to individuals with substance use or co-occurring disorders and their families. Manages care and treatment as well as updates treatment progress reports. Works as an advocate for the client in the community. Participates in treatment planning and case conferences with other internal and external service providers, if applicable. Maintains records of all engagements performed using NCCPD data collection tool.
Flexibility to work a rotating shift and assist with covering on-call responsibilities as assigned. Desired candidates possess a high school diploma or equivalent and five years of experience working in the field of substance use disorder treatment required. CADC/CAADC is preferred, but not required.
