Tampa, Florida, Florida, This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U. S. C. 7402(d), and 7407(d). Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm. D.

) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy). Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i. e. , Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grade Determinations: (a) Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. (b) Assignments: Candidates at this grade level are to be in one of the assignments listed below.

For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Facility Program Manager: Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c.

Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: 1. Post-Graduate Year 2 (PGY2) Pharmacy Residency Program in Critical Care or Internal Medicine. 2. Experience working as an Operating Room (OR) clinical pharmacist for at least two years. 3.

Experience working as a Critical Care clinical pharmacist for at least two years. 4. Experience working as an Internal Medicine clinical pharmacist for at least two years. 5. Experience working in a hospital pharmacy as a clinical pharmacist for at least two years. Uses expertise in medication management in collaboration with interdisciplinary surgical team members. Develops therapeutic plans intended to achieve definite drug therapy outcomes. Anticipates and responds to intraoperative medication requirements. Uses enhanced clinical decision-making skills in performing direct interventions. Responds to operational and surgical scheduling flow to ensure timely delivery of services. May prescribe and adjust medication therapy with an approved scope of practice in accordance with established hospital bylaws for midlevel practitioners. b. When necessary, performs appropriate physical assessment (within the confines of experience and training) when necessary to complete the patients’ assessment and treatment, e. g. , taking pulse rate, measuring blood pressure.

c. Interviews patients to determine medical history and reviews history for potential drug-interactions, poly-pharmacy, therapeutic duplication, non-compliance, adverse drug reactions, and proper compliance of preoperative medications to take or hold. d. Monitors and manages medication therapy including initial assessment of new patients and follow-up of existing patients. Education: a. Educates patients on the proper use and care of their medications b. Assists in the development, enhancement, and operation of resident and student training programs. c. Serves as an effective preceptor, directing and supervising pharmacy residents and students as assigned. Quality Assurance/Administrative: a.

Drafts, reviews, and proposes to Pharmacy Management revisions to Medical Center and Pharmacy Service policies and procedures, especially those regarding perioperative pharmacy operations and safety and quality management. Helps assure department contribution to successful OIG, TJC, and other quality and risk management inspections or audits. b. Works with multi-disciplinary groups and committees as assigned in order to develop solutions to specific issues related to compliance with relevant standards and quality issues. c. Identifies opportunities for improvement in pharmacy processes and conducts quality assurance studies on internal procedures. Analyzes data, identifies trends, prepares, and/or reviews reports involving trend analyses in response to requests by the Pharmacy Service management team. Brings to the attention of the Chief of Pharmacy those areas of possible weakness and performs follow-up in areas of concern as assigned. Research a. Design, develop, collect, and analyze data related to patient outcomes and clinical programs.
