White City, Oregon, Oregon, As the Pharmacy residency program director (RPD), this pharmacist is responsible for the direction and oversight of the Pharmacy residency program, including the preceptors, and is committed to serving as a role model and providing effective training of Pharmacy residents. This position will work closely with the Chief, Pharmacy Service, Clinical and Operational Associate Chiefs of Pharmacy as appropriate at VA Southern Oregon Rehabilitation Center & Clinics (SORCC). Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm. D. ) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy. ) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i. e. , Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: In addition to the basic requirements for employment in paragraph 2, the following criteria must be met when determining the grade of candidates. GS-13 (a) Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. (b) Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities.
The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacists assigned to this position must demonstrate the KSAs identified in subparagraph 3b(3)(b)1 above. a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Pharmacy residency program experience with improvement, readiness, strategies practiced in preparation for and in response to ASHP accreditation standards and surveys. References: VA HANDBOOK 5005/55 PART II APPENDIX G15 The full performance level of this vacancy is GS-13. Physical Requirements: 1. The work requires physical exertion such as reaching, bending, standing, walking, climbing, pushing, pulling, and kneeling. Occasionally may need to lift or carry up to 15 pounds. Use of both hands and fingers for fine motor skills (manipulate and open medication bottles, data entry, etc. ). Due to the nature of the work environment, ability to distinguish basic colors and shades is required.
Near and far vision must be correctable to perform tasks. 2. The work environment involves everyday risks or discomforts and repetitive movements which require normal safety precautions typical in a clinical pharmaceutical setting. 3. The work area is to be adequately lighted, heated, and ventilated. The employee may work with cleaning/disinfecting wipes and rubbing alcohol. 4. Employees may be required to use protective clothing or gear such as masks, gowns, goggles, gloves, or shields to minimize risks in certain circumstances. 5. The work requires direct patient contact; appropriate personal protective equipment is to be used.

Responsible for serving as chair of and facilitating residency advisory council (RAC) meetings. 2. Will serve as a positive role model for Pharmacy practice residents through demonstrated leadership, professionalism, contribution and advancement to the profession of Pharmacy. 3. Responsible for coordination of residency recruitment events and prospective Pharmacy practice resident interviews and interview scoring. 4. Responsible for coordinating Pharmacy practice resident on-boarding, initial and on-going orientation and training, provision of overall guidance, development, and evaluation of professional and personal skills throughout, including conduct issues, and documentation of completed requirements with a goal of successful program completion.
Identifies improvement opportunities and develops corrective action plans when necessary and as appropriate. 5. Responsible for defining, developing, and implementing criteria for Pharmacy residency program preceptors including evaluation, eligibility, qualifications, skills assessment, suitability, professionalism, and development to ensure a commitment toward teaching and developing and advancing residents, the residency program, and Pharmacy practice. 6. Responsible for leading and collaborating with Pharmacy Service and health care system leadership to ensure continuous Pharmacy residency program planning, improvement, readiness, and execution strategies are established and practiced in preparation for and in response to ASHP accreditation standards and surveys. 7. Responsible for assuring residency training is conducted in accordance with accreditation standards of the American Society of Health-System Pharmacists (ASHP and Office of Academic Affiliations (OAA). 8. Collaborates to ensure timely submission of funding requests for recruitment events, annual ASHP accreditation fees and OA annual funding request for Pharmacy residency positions. Clinical Pharmacy Practitioner (CPP): (60%) 1.

Clinical – Direct Patient Care Activities that require a scope of practice: a. Core functions include face-to-face, video visit, and telephone encounters with referred patients, for comprehensive medication management in practice areas identified in their individual scope of practice. b. Functions as a provider with a scope of practice to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. c. Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient’s drug therapy. d. Follows-up with patients on lab or test results to discuss the plan of therapy, i. e. changes in medication therapy, monitoring, and additional testing requirements.
e. Clinical pharmacy specialists will determine if a referral to the physician is necessary when disease progression occurs or adverse drug events require treatment interventions outside of the CPP scope of practice.
