Houston, Texas, Job Summary The Pharmacy Technician Transition Specialist’s priority is to deliver optimal medication management across all dimensions of care at all times and ensure world class service recognized for excellence. The Pharmacy Technician Transition Specialist is responsible for actively engaging patients, fulfilling prescriptions for accuracy and safety, and coordinating medication delivery and medication education to patient’s bedside prior to discharge. The Pharmacy Technician Transition Specialist simplifies the discharge medication process, making it easy, so that patients can focus on what really matters – getting better and staying healthy. The Pharmacy Technician Transition Specialist provides customer service excellence to all patients, creating unforgettable patient experiences. The Pharmacy Technician Transition Specialist will work closely with individuals from pharmacy, nursing, and medical staff to ensure prescriptions are filled and delivered, with education being provided on time before patient is discharged from the hospital. Coordination of efforts with nursing and physicians is critical. The Pharmacy Technician Transition Specialist recognizes physicians and nurses as essential partners with shared goals of delivering the remarkable patient experience. Under the direct supervision of a licensed pharmacist, Pharmacy Technician Transition Specialists will actively engage patients throughout their hospitalization and facilitate processing and delivery of discharge prescriptions to the bedside. The need for absolute accuracy and precision in all aspects of the job is essential to ensure patient health and safety. During down time, the Pharmacy Technician Transition Specialist is expected to work with inpatient staff and managers to support inpatient pharmacy operations.

Pharmacy Technician Transition Specialists may also perform medication histories for new patients in the Emergency Center and admitted to the hospital. Under the general supervision of the clinical pharmacist, they shall provide complete and accurate information for each medication, e. g. , drug name, dose, route, frequency, indication, and date/time last dose was taken. The Pharmacy Technician Transition Specialist will utilize available resources for medication history collection including but not limited to: patient/family interview, medication lists, outside facility medication administration records, outpatient pharmacy records, and physician office records. They will routinely verify and update patient allergies, adverse drug reactions, preferred pharmacy and primary care physician information in the electronic medical record. Medication histories are to be completed in the electronic health record and sent to the pharmacist for cosignature in an accurate and timely manner. The Pharmacy Technician Transition Specialist will assist the pharmacist by ensuring patients identified for hospital admission have a completed medication history. Education materials may also be prepared as necessary. Minimum Qualifications Degrees: Associate’s degree (preferred) School Education: High School Diploma Or GED Licenses & Certifications: a) Registered Pharmacy Technician by Texas State Board of Pharmacy & in good standing b) Medication Therapy Management Certification: Medication History (preferred) Experience: Two (2) Years of Work Experience as a Pharmacy Technician; with a minimum of 1 year of experience at hospital/infusion pharmacy and 1 year of experience with outpatient pharmacy Languages: Spanish Fluent (Preferred) Communication Skills: a) Above Average Verbal Communication (Heavy Public Contact) : Strong communication (written & verbal) and analytical skills, as well as the ability provide education b) Exceptional Verbal (Public Speaking) c) Writing/Correspondence Proficiencies: a) MS Office b) Typing between 20-29 words per minute: 25 WPM Job Attributes Knowledge/ Skills/ Abilities: a) Mathematics b) Medical Terminology Knowledge Work Schedule: Weekends, Holidays, Flexible Other Special Requirements Other Requirements: a) Demonstrates professionalism, confidence, strong organizational skills, efficiency, initiative, resourcefulness, and an ability to work in a fast paced, rapidly changing environment.
