Dedicated to improving the health and well-being of patients, this position collaborates with existing PCPs, the HbMC interdisciplinary team, and other health professionals, to develop and realize patients’ goals of care in all phases of longitudinal care. D. ) or Doctor of Osteopathy (D. O. ) Active, unrestricted license to practice medicine in the State of New York Board Certified or Board Eligible in Internal Medicine, Geriatrics sub-specialty, Family Practice or Emergency Medicine Experience providing medical care to geriatrics and/or seriously ill patients Current DEA license 2+ years of clinical experience Access to reliable transportation required Preferred Qualifications: Home-based clinical care experience Familiarity with Medicare Guidelines, CPT and ICD-10 codes Knowledge of clinical standards of care Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines Familiarity with Medicare Guidelines **PLEASE NOTE** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire/rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments. New York Residents Only: The salary range for New York residents is $ 220,780. 00 to $308,300. 00.
Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
