About the role:
Overall responsibility for the oversight and administration of the PACE program, which includes the effective planning, organization, administration, and evaluation of the organization’s operations. This includes ensuring that appropriate personnel perform their functions within the program This position is responsible for managing all aspects of a multi-site PACE program, which consists of primary care medical clinics, 24-hour care delivery, preventive services, a provider and healthcare network that encompasses all Medicare and Medicaid services, therapeutic services, PACE centers and alternative PACE centers, home care, transportation, and Quality Assurance and compliance activities. Duties include ensuring that the program complies with all Centers for Medicare and Medicaid Services (CMS) guidelines, ensuring the financial viability/continued growth of the program, managing business relationships with all contracted providers, and managing the state and federal relationships surrounding the PACE program. This position must have the ability to travel on a routine basis between PACE locations and other Healthcare Partners in the KY service area, as needed.
Supports the standards of quality, patient, and team member safety, and contributes to the total organizational culture and philosophy. Cultivates a supportive and inclusive work environment for all team members through continuous development, modeling inclusive behaviors, and proactively managing bias.
Health Care Professional with a master’s degree in Health Administration, Public Health or clinical discipline preferred; or a combination of education, significant management/leadership experience; experience with PACE or related programs a plus.
Must possess strong financial and budget management skills.
Must have at least one year of experience working with the frail and elderly.??
Food handling permit required, to be completed upon hire.?
Previous experience in home care or post-acute setting preferred, with three to five years’ proven growing, developing and expanding programs/services.
Proven ability to lead personnel, cultivate a positive work culture and build effective teams.
Complies with accepted professional standards of practice, rules and regulations.
Excellent observation abilities, critical decision-making, and communication skills, both written and verbal.
Strong understanding of geriatric patient population, underserved/underrepresented communities and the seriously ill, including the long-term care eligible population.
Is flexible and cooperative in fulfilling all obligations.
Is self-directed with the ability to work with little supervision.
Must be a licensed driver with automobile that is insured in accordance with state and/or organization requirements and is in good working order.
