Schedule:  Full Time 40 hours per week Shift :  Evenings, 1:00pm to 9:30pm Mon-Fri, however some weekends may be required as needed based on staffing needs. The community you’ll be caring for:  Faith based and Mission driven agency Flexible scheduling One-on-one patient care in the patient’s home environment Joint Commission Deemed Status accredited agency Competitive pay with no state income tax Commitment to workplace wellness and exceptional health and benefits offerings   The role you’ll contribute:  The Triage RN is responsible for providing service to all patient and family/caregiver telephone calls received and coordinates/dispatches appropriate level of care to patient.   The Triage RN supports and coordinates patient requests in order to ensure that the appropriate care is given in a timely manner as deemed necessary.     The value you’ll bring to the team:  Provides nursing services to all patients and families of Hospice of the Comforter via telephone support.   ·          Receives and responds to all telephone calls and/or electronic pages made by patients, caregivers, doctors, or others regarding a patient’s needs or changes in condition.

·          Assesses the patient’s needs, prioritize requirements, and determines what actions should be taken. ·          Coordinates and directs staff for urgent patient visits. Sends staff to provide patient care, make further evaluations, deliver and/or administer medication and medical supplies, or follow other orders. ·          Communicates pertinent patient and employee information to interdisciplinary teams and Patient Care Managers on a daily basis. ·          Consults the Nursing Manager regarding unusual situations and potential risk management issues, and confers with before initiating Crisis Care or General Inpatient (GIP) status. ·          Assesses the need for other Hospice of the Comforter or community resources and notifies appropriate parties of need for response. May call ambulance service, funeral home, law enforcement, psycho-social workers, or others to activate services in response to patient needs. ·          Documents patient contact and activity into computer system and maintains accurate logs, dispatch of staff and others in response to patient care for all calls received. Qualifications The expertise and experiences you’ll need to succeed: Minimum qualifications :  ·          Graduation from accredited nursing program, equivalent to one year of practical training with certification as a Registered Nurse. An equivalent combination of education and experience may be substituted at the discretion of the President/CEO.

·          Minimum of 3 years experience as an RN required.   ·          Clear/active Florida Registered Nurse (RN) license is required ·          Basic Life Support (BLS) required. ·          Hospice or home health, acute care nursing experience is preferred. ·          Certified Hospice and Palliative Nurse Certification are preferred.
