RN Navigator

Morristown/NJ/USA, Overview Atlantic Health System is seeking a full-time RN Navigator for the Atlantic Medical Group Physical Medicine and Rehab Center. Responsibilities Essential Functions: Serves as patient and family advocate Coordinates the timely scheduling of appointments to expedite the plan of care and to promote continuity of care Schedules, re-schedules, and cancels appointments for the appropriate outpatient service Assists in answering patient calls, initiate call backs and follow-up to meet the needs of the patient Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment Other duties as assigned Qualifications Education: BSN preferred Valid New Jersey Nursing License Required Minimum of two years of acute care/physician practice experience EPIC experience preferred Qualifications: Must have 3 + yrs. experience, outpatient preferred About Us Atlantic Health System aims to deliver the highest quality, safety and care combined the best experience for our patients and their families.
