Position Information: Become an internal “Travel” Nurse within one of WakeMed’s newest and most modern Outpatient facilities in the surrounding Raleigh area. 13-week assignments, with option to renew, full-time, flexible work week, outstanding compensation package, and if desired a means into a perm opportunity. Responsibilities:
* The Ambulatory Care / Primary / Urgent Care Nurse is a Registered Nurse (RN) responsible for providing nursing care management services to assigned populations
* Utilizes a collaborative interdisciplinary approach to anticipate, integrate, and coordinate the patient’s plan of care and maximize the ability to remain in a community setting
* The goal of the intervention is to meet the needs of patients in order to optimize clinical and resource outcomes, and to monitor the effectiveness of the plan and its outcomes
* In collaboration with the physician, the nurse identifies at risk patient populations who require care management services
* Provides bio-psychosocial assessments to identify patient needs and to assist in the development of the plan of care
* The Ambulatory Care Nurse serves as a patient/family advocate and acts as a communication link between the physician, patient, family, outpatient and inpatient health care team, as well as community resources, to reduce redundancy and fragmentation of care Experience Requirements: 1 Year Outpatient Nursing Required Education Required: Graduate Nursing Required – Or College Courses Nursing Required – With Required Licensure/Certification Licensure/Certification Requirements: Registered Nurse Required Hours of Work: Any Weekend Requirements: As Needed Call Requirements: As Needed
