Schedule: Full Time Days Shift : 8am-4:30pm Location: Trina Hidalgo office: 101 S WESTMORELAND DR ORLANDO FL 32805 The community you’ll be caring for: AdventHealth Orlando The role you’ll contribute: The Outpatient Clinical Social Care Manager-Clinics is a key member of the healthcare team working with high-risk patients and their families to help manage the difficulties of a health or life problem. Utilizes professional social work skills to assess patients for psycho-social and economic barriers that interfere with the attainment of healthcare goals. Works closely with Case Managers, Physicians/Providers, Nursing staff, healthcare team, spiritual support and others, as appropriate, to advocate for the patients and families with counseling services, crisis intervention, stress management, patient and parent coping (as appropriate), bereavement, abuse and neglect issues and continuity of care. Will be in an independent role and responsible for care management of two different cardiac (and potentially other disease states) clinics with diverse patient populations. The value you’ll bring to the team: · Applies knowledge of local, state, and federal programs to assist with appropriate referrals.
· Perform duties and responsibilities in an independent role with oversight of two different cardiac clinics with diverse patient populations. · Actively contributes to the social work program supporting patients, families and physicians/providers. · Knowledge of community resources, medication patient assistance programs to facilitate optimization of whole patient care and removal of barriers. · Independent development of programs to support families or patients dealing with complex medical issues. Qualifications The expertise and experiences you’ll need to succeed : Minimum qualifications : · Master’s Degree in Social Work · Two years of related experience Preferred qualifications: · Experience in ambulatory care social work · Experience in counseling patients (per patient demographics) and families · Licensed Clinical Social Worker This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
