Valhalla, New York, Job Summary: The Transplant Data Coordinator works closely with the Director for Data Compliance and is responsible for obtaining, tracking, and maintaining transplant specific clinical and demographic data for internal and external purposes. The data manager is also responsible for data collection, data entry and compliance with all reporting requirements specified by Centers for Medicare and Medicaid Services (CMS), United Network for Organ Sharing (UNOS), New York Department of Health (NY DOH), Join Commission (JC) and other regulatory agencies. Responsibilities: Collection of clinical and demographic data for transplant waitlist candidates, transplant recipients, transplant living donors, and post-transplant/donation follow-up via chart review, medical record requests from outside facilities and/or discussion with transplant coordinator. Accurate and timely entry of clinical and demographic data for transplant candidates, transplant recipients, and living donors directly into the United Network for Organ Sharing’s national transplant registry system in accordance with policies and bylaws. Entry and validation of data entered in transplant patient management system for patient management and research purposes. Assist Director of Data Compliance in preparation and maintenance of transplant research database; data collection, validation, analysis and preparation for Transplant Quality Assurance / Performance Improvement program; and in audits of data for internal and external registries. Completes required data reporting to the End Stage Renal Disease network. Completes all responsibilities above in accordance with institutional policies and procedures relating to transplant data and protected health information. Maintains patient / employee / institutional confidentiality in the management data. Demonstrates a professional, courteous and respectful attitude in dealing with patients / families, other transplant team members and outside healthcare professionals.

Performs all other duties as assigned. Qualifications/Requirements: Experience: Minimum two years of experience in healthcare related field, required. Experience in data collection/entry, medical chart abstraction, or related field strongly preferred. Knowledge and/or experience in organ transplantation desirable. Expertise with medical terminology/records and electronic medical records systems preferred. Strong computer skills: MS Office (Word, Excel, PowerPoint). Education: Associates degree or equivalent, required. Bachelors preferred. Licenses / Certifications: About Us: WMC Advanced Physician Services PC
